TB-500 is a synthetic peptide related to thymosin beta-4, a natural protein involved in cell movement and tissue repair. It is popular for injury recovery, but there are no controlled human trials of TB-500 injections for sports injuries, and as of October 2026 it is neither FDA-approved nor formally on the pharmacy compounding list. Dr. David Nazarian, a board-certified internist in Beverly Hills, reviews the evidence with patients and focuses on treatments with stronger support.
What Is TB-500?
Thymosin beta-4 is a protein found in most human cells. It binds actin, a structural protein cells use to move, and it appears in high amounts where tissue is being repaired. TB-500 is a lab-made peptide modeled on the active region of thymosin beta-4. It is marketed mainly as an injection for muscle, tendon and ligament recovery, and is often promoted alongside BPC-157.
What the Research Shows
Animal and laboratory studies suggest thymosin beta-4 can support cell migration, new blood-vessel growth and wound repair. Human research has focused on thymosin beta-4 itself, not TB-500, in specific settings such as eye-surface healing and skin wounds, with mixed and modest results.
There are no published randomized trials showing that TB-500 injections speed recovery from muscle strains, tendon injuries or joint problems in people. Claims about faster healing, flexibility or hair growth rest on animal data and anecdote.
Possible Risks and Unknowns
- Safety is not well characterized. Appropriate dosing, side effects and long-term effects have not been established in human trials.
- Cell growth signals. Because thymosin beta-4 promotes cell migration and blood-vessel growth, there are theoretical concerns for people with a current or past cancer.
- Unregulated products. Online products labeled for research use may not contain what the label states.
- Prohibited in sport. TB-500 and thymosin beta-4 are banned by the World Anti-Doping Agency.
FDA and Compounding Status (October 2026)
- TB-500 was placed in FDA Category 2 in 2023 because of safety concerns for compounding.
- It was removed from Category 2 in April 2026 after the nomination was withdrawn. That did not make it approved or compoundable.
- In July 2026 the FDA’s Pharmacy Compounding Advisory Committee voted 8 to 6 to recommend adding TB-500 to the 503A bulk substances list, reviewed for wound healing.
- The FDA has not yet completed the rulemaking needed for routine compounding.
We will update this page when the FDA acts.
A Practical Approach to Injuries
Healing depends on getting the diagnosis right. Dr. Nazarian starts with a history and examination, uses musculoskeletal ultrasound in the office when imaging will change the plan, and coordinates MRI or specialist referral when needed. Evidence-based steps often include a progressive rehabilitation program, correcting nutritional or hormonal factors that slow healing, sleep optimization and, for selected cases, regenerative procedures described on our regenerative medicine page.

TB-500 Evaluation in Beverly Hills and Los Angeles
Patients from across Los Angeles, including Beverly Hills, Bel Air, Brentwood, Century City, West Hollywood and Santa Monica, see Dr. Nazarian at our office at 9301 Wilshire Blvd in Beverly Hills.
- Consultation. Your injury or recovery goals, training load, prior treatments, medications and any peptides already used.
- Examination and imaging. A focused exam and in-office musculoskeletal ultrasound when imaging will change the plan.
- Baseline labs. Typically a complete blood count, comprehensive metabolic panel, HbA1c, hs-CRP, vitamin D and ferritin.
- Safety screening. Personal and family cancer history and age-appropriate screening, since thymosin beta-4 promotes cell migration and blood-vessel growth.
- Benefits and risks discussion. The research, the unknowns, current FDA status and all treatment options.
- Follow-up. Repeat labs and progress tracking to measure your response.

Typical Peptide Treatment Schedule
When a peptide is part of your plan, Dr. Nazarian typically prescribes it at night, five nights a week, Monday through Friday, with weekends off, for an eight-week course followed by a two-week break so the body does not adapt to it. The specific peptide, dose and length of treatment are discussed individually and adjusted to your labs and response.
Frequently Asked Questions
Where can I get TB-500 in Los Angeles?
Begin with a physician evaluation. At My Concierge MD in Beverly Hills, Dr. Nazarian reviews your history, runs baseline tests, explains benefits, risks and FDA status, and discusses treatment options, including TB-500 through licensed compounding pharmacies as that access is established.
What tests do I need before starting TB-500?
A blood count, metabolic panel, inflammation markers and a cancer screening review, plus imaging when an injury is involved.
Is TB-500 better than BPC-157?
They work differently. TB-500 is based on thymosin beta-4, which helps cells move into damaged tissue. BPC-157 is studied mainly for tendon, ligament and gut healing. Neither has been compared head to head in human trials.
How much does TB-500 treatment cost in Beverly Hills?
Cost depends on the evaluation, lab tests and treatment plan. A new-patient consultation with Dr. Nazarian is $450. Lab tests and any treatment are priced separately, and our team gives you those costs before testing begins.
Is TB-500 the same as thymosin beta-4?
Not exactly. TB-500 is a synthetic peptide based on part of thymosin beta-4. Human studies of thymosin beta-4 do not automatically apply to TB-500.
Is TB-500 FDA-approved?
No. It is not approved for any use, and it is not yet formally on the list of substances pharmacies may compound.
Why do athletes use TB-500?
It is promoted for faster recovery, largely based on animal studies and personal reports. It is prohibited in tested sports.
What should I do if I have already used it?
Tell your physician, including the source and dose. That helps us interpret symptoms and lab results and plan safe next steps.
